Life Generations Healthcare LLC (“LGH”) announced today that it is notifying individuals of a recent privacy incident.

LGH recently discovered that, from May 24 to June 13, 2023, someone outside of LGH temporarily accessed certain LGH employee email accounts without authorization. Upon learning of the situation, LGH promptly contained the incident by securing the accounts to prevent further access and began an investigation into the incident. LGH also hired a leading forensic security firm to assist with the investigation into the incident and confirm the security of LGH computer systems and network. This incident was limited to a small number of email accounts and did not impact LGH medical record systems.

On October 4, 2023, the investigation determined that the incident could have resulted in the unauthorized access of some individuals’ personal information. The type of information differs for each individual but could have included information such as name, address, date of birth, medical information, health insurance information, Social Security number, driver’s license number or state ID, and financial account information. LGH has begun sending written notifications to potentially involved individuals for whom it has contact information and arranged for complimentary identity theft protection services for those individuals whose Social Security numbers and/or driver’s license numbers were involved in the incident.

Involved individuals should refer to the notice they will receive in the mail regarding steps they can take to protect themselves, should they feel the need to do so. LGH has no reason to believe that any personal information has been misused for the purpose of committing fraud or identity theft, but as a precautionary measure, individuals should remain vigilant to protect against potential fraud and/or identity theft by, among other things, reviewing their account statements and monitoring credit reports closely. If individuals detect any suspicious activity on an account, they should promptly notify the financial institution or company with which the account is maintained. They should also promptly report any fraudulent activity or any suspected incidents of identity theft to proper law enforcement authorities, including the police and their state’s attorney general.

Involved individuals may also wish to review the tips provided by the Federal Trade Commission (“FTC”) on fraud alerts, security/credit freezes and steps that they can take to avoid identity theft.
