We provide a full range of tailored nursing care and social services for our patients and residents. Thanks to our clinical capabilities, we accept patients with more complex medical issues than a traditional nursing home. Whether you need short- or long-term care, we provide a safe, compassionate environment that encourages dignity and independence. Rehabilitation Programs
Our rehabilitation program is comprehensive and includes physical, occupational, and speech therapy. We have helped many patients achieve outstanding results, and our staff has experience caring for people recovering from numerous conditions, including cardiac, orthopedic, neurological, and other illnesses.
We provide inpatient therapy services seven days a week. Upon admission, our team will consult with your physician and create a personalized therapy program specific to your needs and desires.
Sub-Acute Unit
Walnut Creek Skilled Nursing & Rehabilitation is pleased to provide a sub-acute care program for residents recently released from the hospital. Our main goal is to reduce the risk of readmission to the hospital as well as weaning our patients off of tracheostomy and ventilators to get them to a lower level of care. We wish to promote quality of life to each patient.
We seek to help our patients in the recuperation process while helping them achieve their maximum capabilities to return home as quickly as possible.
Our nurses and respiratory therapists are available 24 hours a day. We also have our therapists, social workers, dietitians, and case managers available to fit each individual’s schedule. Our facility also has an internal medicine physician that comes to the unit every other day, along with a pulmonologist that comes weekly. Our team is very dedicated to our patients’ well-being.
Sub-acute care is for residents who are medically fragile and require special services. These services include the following:
- Inhalation therapy
- Tracheotomy care
- Intravenous tube feeding
- Complex wound management
- Spinal injuries
- Aggressive weaning with our pulmonologist’s recommendations
